Admission Information

The admission process can be a bit overwhelming, but our teams work with you, your loved ones and the referring hospital to make the transition as smooth as possible, and without interruption of your treatment.

Upon admission, you or your loved one will be assigned a room and treated without regard to race, color, creed, age, handicap, national origin or payment classification.

During admission, or closely thereafter, please bring:

  • Identification card(s)
  • Insurance cards and policies
  • Medicare or Medicaid cards
  • Living will, power of attorney or durable power of attorney 
  • Health surrogate appointment
  • Copy of an organ donation request card
 
 

Personal Items

During your stay, only a few personal items are needed; we will supply the rest! We recommend you or your loved one bring items such as deodorant, toothbrush, shampoo, robe and slippers. If you require glasses or dentures, please be sure to pack them in appropriately labeled storage containers. Electric appliances are not allowed unless they are approved by our Administration Department.

While we do our best to secure our hospital, we strongly suggest you leave valuables at home as we are not responsible for valuables or money kept in your room.

Bedside Care Team Meetings

Recovery takes a team. And the most important member of that team is you!

Throughout your recovery journey, caregivers from all relevant departments will gather at the bedside at least weekly, or more often depending on need, to discuss goals, milestones, treatment, and progress. We invite patients and families to participate in these meetings and make their voices heard. The best care plan is one that you understand and helped create.

Depending on your condition, your interdisciplinary care team may include: 

  • Physicians with specialties in pulmonology, cardiology, infectious disease, etc.
  • Critical care nurses
  • Respiratory therapists
  • Certified wound care coordinators
  • Physical, occupational, or speech therapists
  • Dietitians
  • Pharmacists

These skilled clinicians work together to develop a care plan customized for you and your needs. 

 
 

Patient Relations Representative

At Solara, we keep you and your family at the heart of what we do. That's why each of our patients has access to a Patient Relations Representative who will meet with you, introduce you to our facility, and answer any questions you may have. Your Patient Relations Representative serves as bridge between you, your care team, hospital leaders and all our teammates to improve any and all aspects of your stay.

Financial Matters

To guide you and your loved ones through the financial process, we have a dedicated business office staff member to handle all insurance billing. Should you have questions on the financial details or insurance coverage, we are more than willing to arrange a meeting.
